FcRn(新生儿Fc受体)

背景

FcRn与配体IgG和白蛋白的作用位点不同
长期以来,IgG一直被认为是唯一一类从母体主动转移到后代,从而产生短期被动免疫的抗体,而这种特殊的IgG运输正是由FcRn完成。1972 年,Jones 等首次在新生大鼠肠道发现通过转运母体IgG给新生儿的受体,新生儿 Fc 受体(Neonatal Fc receptor, FcRn)由此命名而来。FcRn在妊娠期和哺乳期表达起到跨越胎盘屏障和肠道运输 IgG 的作用,在整个生命周期的多种组织细胞都能够被检测到,主要功能是维持着血清中IgG 和血清白蛋白的水平、调节IgG在组织中的分布。
FcRn 是由FCGRT和B2M两个亚基组成的异源二聚体。FCGRT具有α1、α2和α3三个胞外功能区,1个跨膜区和1个胞浆尾区(有研究推测由44个氨基酸残基组成的胞浆尾区,可能含有介导胞内途径的信号),分子量为40~50 kDa,称为α链;B2M分子量为14 kDa,称为β链,两条链以非共价键的形式结合在一起;FCGRT必须和B2M装配后才能发挥转运作用。有研究表明,FcRn与IgG和血清白蛋白的结合位点并不相同,因此FcRn与IgG 结合不会受到血清蛋白的干扰。
IgG在人体内半衰期长达2~4周,影响半衰期主要归因于FcRn介导的再循环机制,而这种机制是通过IgG的Fc片段和血清白蛋白与FcRn的pH依赖性结合而实现。 在酸性条件下(pH6.0-6.5),FcRn结合IgG,在中性及弱碱性条件下(pH7.0-7.5)发生解离。具体来看,内皮细胞通过形成内吞小泡,将IgG摄取后形成酸化内体,IgG与FcRn结合形成IgG-FcRn复合物。IgG-FcRn复合物通过再循环内体转运到细胞表面,生理条件如pH7.4下,IgG-FcRn复合物发生解离,IgG重新释放至血液循环中。通过这种受体介导的再循环机制,FcRn有效保护IgG避免在溶酶体中降解,从而延长IgG的半衰期。而酸化内体内未与FcRn结合形成IgG-FcRn复合物的IgG,则在溶酶体内发生降解。研究表明,FcRn介导的IgG回收率比IgG产生率高出42%,这表明相比于产生IgG, IgG的回收是维持着人体IgG浓度的主要过程。
除了半衰期评估,FcRn作为靶点的潜力也相当巨大,主要因为FcRn的高亲和力尤其对IgG介导的如重症肌无力、类风湿性关节炎或寻常天疱疮等自身免疫性疾病造成有害影响。靶向FcRn,抑制FcRn循环可使IgG分解代谢增强,导致整体IgG和致病自身抗体水平降低,有望降低所有IgG引起的自身免疫异常。有研究证实,与重症肌无力的治疗性血浆置换疗法相比,以FcRn靶点的治疗方法可以提供更快速和更具选择性的IgG降低效果。另外,与临床使用的许多常规疗法相比,这种治疗方法优势突出也体现在IgA、IgD、IgE和IgM不依赖于FcRn介导的循环,因此它不会导致广泛的免疫抑制,为FcRn靶向的安全性和精准特异性提供了基础。因此,靶向FcRn的药物,并非像常规靶点一样将FcRn作为治疗干预的直接靶点,而是利用其细胞药物传递的优势,被称为可一网打尽所有自身免疫性疾病的“革命性疗法”,具备极高的潜力。
